Physiotherapeutic procedures in the treatment of postpartum stress incontinence

This bachelor thesis focuses on physiotherapeutic procedures in the treatment of postpartum stress incontinence. It is divided into theoretical and practical parts. One of the aims of my thesis is to describe the therapeutic approaches that can be used to treat postpartum stress incontinence. Then, in the theoretical part, I focus on describing the postural changes caused by pregnancy and the period during the postpartum period. The last aim is to describe the effect of the chosen therapy on the woman's posture and the impact on quality of her life. In the theoretical part, information on the current state of the problem is given, as well as the anatomy and physiology of the organs in the body. The following chapter deals with stress incontinence, specifically its classification, epidemiology, etiology, diagnosis and treatment. The next chapter deals with the changes on a woman's body during pregnancy, and last but not least, there is a chapter discussing pelvic floor problems and the possibilities of physiotherapeutic procedures to affect (not only) the pelvic floor muscles. The practical part describes the methodology of the work, the examination content and the results are noted in the form of case reports. In total, there are case reports of 2 probands, who were at different times after childbirth and suffered from different degrees of stress incontinence after childbirth. For 10 weeks the probands practiced the given exercises, a description of which can be found in one of the chapters. At the same time, their examples can be seen in the photographs in the booklet, which is included in the appendices. The work can be an inspiration for physiotherapists who work with the pelvic floor area or specifically with women after childbirth. It can also be useful to anyone dealing with a problem in the pelvic floor area. The booklet can be the first step in solving it, as it contains educational information as well as basic exercises for the pelvic floor muscles.
Myofascial pain syndrome in the pelvic area

Myofascial pain syndrome (MFBS) in the pelvic region is characterized by the presence of trigger points (TrPs) in the pelvic floor (PF) muscles and pain that is localized directly in the pelvis but simultaneously radiates to surrounding structures. This pain is often difficult to distinguish from internal complaints, which are often mimiced or accompanied by the pain. This bachelor's thesis addresses the subject of pain in the pelvic region and TrPs, which are related to myofascial structures, particularly PD. In the theoretical part, it aims to bring together the existing knowledge on MFBS with a narrower focus on the pelvic region and to present a comprehensive examination procedure and treatment options. The examination of PD in MFBS should be comprehensive, including history, palpation, dynamic tests and pelvic region specific tests. Therapy is initiated with manual techniques (MT), includes therapeutic physical education (TPE) methods to integrate the pelvis into global movement patterns. Training awareness of PD and the ability to relax it play an important role as well. The effect of body position on pressure in anus during relaxation and contraction of the pelvic floor muscles. Pilot manometric study

This Research thesis is focused on the influence of body position on the function of pelvic floor muscles (PFM). Anatomical structures, kinesiology and neurophysiology of the PFM are described in the theoretical part of this thesis. It also discusses the methodology used for objective evaluation of the PFM. For the practical part of this thesis, 30 healthy subjects (aged 20 - 30 years, 15 male, 15 female) where chosen via a questionnaire. The subjects were examined via the method of anorectal manometry. The goal of the examination was to measure the distribution of a) resting pressure in the anal canal in selected positions, b) pressure during voluntary maximal contraction of PFM and sphincters in selected positions, c) pressure during 20 seconds of voluntary contraction. Selected positions are: lying on the back; lying on the back with legs elevated and held in "three flexion"; kneeling on all fours with palm support; kneeling on all fours with elbow support; squatting, and standing. Results: a) The resting pressure is influenced by the body position. Highest resting pressure was observed in squatting and standing positions, lowest resting pressure was measured on subjects while kneeling on all fours with elbow support. Functional disorders of the locomotor system in patients with primary dysmenorrhea

The thesis deals with primary dysmenorrhea (PD) in relation to the locomotor system. The aim of the the sis was to compare the frequency of selected joint blocks and the incidence of constitutional hypermobility in PD patients in comparison with the control group. The patient group included 9 women with PD symptoms and the control group consisted of 10 women without these symptoms. The incidence of atlanto - occipital joint block within the patient group was significantly higher than within the control group (p < 0,05). The total number of stuck ribs was also considerably higher in PD patients (p < 0,01). Pain on palpation on the top of the coccygeum apex was significantly higher within the patient group (p < 0,001). Patients with PD did not show significantly different results in Beighton score test for constitutional hypermobility assessment when compared with the control group. Another aim of the thesis was to judge the effect of physical therapy on PD symptoms. For three months, the group of 9 patients had been encouraged to do regular exercises with the impact on postural stability and diagnosed functional disorders of the locomotor system by means of physical therapy. Education of women with functional disorders of the motion system to the physiological activity of the pelvic muscles using physiotherapy practice

This work deals with pelvic floor muscle issues. The topic is Educating woman with functional disorders of the locomotor system for the physiological activity of the pelvic floor using physiotherapy techniques. The first part focuses on the anatomical relationships in the pelvic area in terms of single organs, breathing mechanics, pelvic floor work and abdominal musculature. Further chapters are about pregnancy, childbirth and the postpartum period in relation with pelvic bottom and dysfunction of the pelvic floor mechanism. Theory of functional disorders has been explained as a result of the changes woman's body goes through. The aim of the theoretical part was to summarize knowledge about the function of the pelvic floor, dysfunction mechanism and the possibility of a therapeutic effect on functional disorders of the musculoskeletal system. The second part has developed the methodology and therapeutic procedures for two research files with respect to PF activity as part of the deep stabilization system. The first group were women before childbirth in the 2nd and 3rd trimester of their pregnancy; the second group consisted of women after childbirth - during the postpartum period. Characteristic for both research files were the presence of functional disorders of the musculoskeletal system. For the research were used qualitative methods of data acquisition; case history, observation, input and output kinesiology analysis and interview. The main aim of this work is to determine the rehabilitation plan for women in the 2nd and 3rd trimester of pregnancy and the determination of the rehabilitation plan for women in childbed. Both rehabilitation plans contain special set of PF exercises, which include education for their proper implementation. Selective clients have opportunity to insight into the understanding of this muscle group work, not only useful during exercises, but even during every day activities. The Bachelor thesis can be used in physiotherapists' clinical practice. Also, as a source of information for professionals and the general public.
